Instruction:

- Use Form 1 if you have access to Service Desk application
- Service Now, to enter a request for iNYP access for your
employee.

- Use Form 2 to request additional iNYP privileges for a
user.

Only directors (asst/assoc.), chairs of clinical
departments, and physicians in the hospitals and the
universities can request additional clinical privileges for the
employees they manage. Basic access is determined by an employee's
official title, department, and workplace. Note that the request
for additional privileges by itself does not guarantee access.
